We are delighted to have you join us and pleased to be able to offer the Credentialing course and support you in becoming a Diplomate with the Canadian Academy of Pain Management.  

Note: To register for the course you must be a current CAPM member (regular membership fee).

REQUIREMENTS


To be considered to the meet the requirements of the course below is the checklist. The certificate will be sent once all requirements are met.

REQUIREMENTS CHECKLIST:

1) NEEDS ASSESSMENT QUESTIONNAIRE:
A needs assessment questionnaire will be emailed out to all participants before the course starts. 

2) SESSION SURVEYS:
For each course day a survey will be sent for completion.

3) EXAM:
The exam will be sent to you after the course. Participants will have 1 week to complete.

4) DOCUMENTS: 

The documents should be submitted in one PDF file and submitted to the CAPM office: krista@eismanagementgroup.com

  • Submitted a copy of my licensure
  • Submitted a copy of my highest degree
  • Submitted my CV
  • Submitted 3 reference letters

 

5) POST REFLECTIVE SURVEY: 

The final post reflective survey will be sent after the last course day.

All fees are subject to HST. There is no option to take single days for this Virtual Course. The full course is the only option for 2025. If you miss a day in the course you can make up the course day in the next course series dates. If you choose the student fee proof of student status will be required from the CAPM office. Cancellation Policy
Cancellations will be accepted until May 1st subject to a $60.00 cancellation fee. May 2nd— May 13th there are no refunds for cancellations.
